For a mental health referral, you can simply send the Acknowledgement of Ongoing Care form. Schedule a referral consult via email or phone if you would like to discuss your patient’s case with a provider before referring.

Timing depends on scheduling availability and how quickly we receive the information needed for review. Once we have the patient’s medical history and current medication information, we schedule a consultation with our psychiatric nurse practitioner. From there, we answer questions, assess fit, and walk the patient through next steps as efficiently and thoughtfully as possible.

Yes. When a patient has given permission for us to communicate, we work collaboratively with referring clinicians and care teams. Our goal is to keep communication clear and useful, so you know whether the patient was seen, whether treatment appears appropriate, and what next steps are being recommended if care moves forward.

Screening and treatment planning include review of current medications. Ongoing medication management and more general longitudinal care are usually provided by the patient’s established team.
